Please Complete the Faculty/Staff COVID-19 Vaccine Status Attestation Questionnaire

As of June 1, ºÚÁϲ»´òìÈ will require a COVID-19 vaccination for all faculty and staff who access campus over the summer months. All new and returning faculty and staff will be required to be vaccinated prior to the Fall 2021 semester.

Please complete the Faculty and Staff COVID-19 Vaccine Status Attestation Questionnaire to let us know if you have already received the vaccine or to claim exemption from the vaccination requirement for medical or sincerely held religious reasons. It will be helpful to have your COVID-19 Vaccination Record Card on hand while completing the attestation.
